A progress note that separates data from interpretation

Use a consistent structure
Identify the goal, reporting period, measurement method, baseline, recent data, and supports used. Describe progress without implying that the target was met when the evidence does not show it.
An illustrative pattern
“During [REPORTING PERIOD], the learner completed [OBSERVED RESULT] using [SUPPORT]. The baseline was [BASELINE]. Data were collected through [METHOD] on [DATES]. The team will review [NEXT STEP].”
Leave missing data visible
If the notes do not include enough measurements, say so. Do not invent a score, an improving trend, or a reason for an absence. The progress report tool can organize the evidence you provide; you remain responsible for checking it.
