Document the real decision
List the action proposed or refused, the actual reasons, supporting evaluations or records, and the alternatives considered. Explain why alternatives were rejected using information supplied by the team.
Verify required local details
Complete the appropriate safeguards information, assistance contacts, and language-access steps. Do not state that a notice was delivered, a meeting occurred, or a parent agreed unless that actually happened.
